This file review revealed that the laboratory results and report <br /> of findings have not been submitted to PHS-EHD. <br /> In order to evaluate your site, a comprehensive report of the activities completed on site <br /> during and after the tank removal must be submitted to our office for review and approval. <br /> Closure/site certification can only occur after successful remediation activities have been <br /> documented and approved by PHS-EHD. Contaminated sites with incomplete or <br /> unsuccessful remediation will remain on the contaminated site list and will require further <br /> investigation prior to issuance of any certification letter. <br /> Please direct your submittals and questions to Michael Infurna, Senior REHS, at (209) <br /> 468-3454. <br /> Ernest M.
