Valinoti, Assistant Director <br />December 15, 1986 <br />UNDERGROUND STORAGE TANK PROGRAM - PERMIT TO REMOVE OR INSTALL UNDERGROUND <br />STORAGE TANKS <br />The following persons can take out permits to remove or install underground storage <br />tanks: <br />Class A Contractors - Tank removal or installation permitted with no restrictions. <br />Class B Contractors - Tank removal or installation permitted if the tank removal <br />is part of some building construction in progress on the <br />property. The Building Construction Permit Number and the <br />issuing agency shall be recorded on the Environmental <br />Health Tank Removal Permit Application. <br />Class 21 Contractors - Tank removal permitted if the tank removal is part of some <br />building demolition in progress on the property. The <br />Building Demolition Permit Number and the issuing agency <br />shall be recorded on the Environmental Health Tank Removal <br />Permit Application. <br />Class 61 (D-40) Contractors - Removal or installation of service station <br />equipment only (including tanks). <br />NOTE - Workers Compensation and License Number for all contractors shall be on <br />file with San Joaquin Local Health District before a permit is issued. <br />UGT 11 Replaces 7-17-86 <br />
