Airport Way, Stockton CA <br /> The permit(SR0053334)to reprogram the monitoring panel at the above referenced site has been <br /> approved with the following conditions: <br /> Conditions of approval: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD) for approval prior to commencing work. All new addenda supercede <br /> the previous ones. <br /> 3. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL, ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council (ICC),that are applicable to the work being performed, and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested,repaired,or serviced; <br /> and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 5. Before the alarm history is cleared, a current printout must be created and available for <br /> EHD inspection. Inspection may occur during the functionality test. <br /> 6. Contact Ray von Flue (468-9848)to arrange the following inspections(a 48-hour notice <br /> is required): <br /> a. EHD must witness the functionality of the monitoring system to ensure that <br /> monitoring has been restored to meet the requirements of California Code of <br /> Regulations Title 23 and the Health& Safety Code. <br />
