Airport Way, Stockton <br /> The proposed permit application to remove one 1,500 gallon underground storage tank <br /> at the above referenced location, has been approved with the following conditions: <br /> 1. Eldon -and Judy Kuntz, new property owners, must be notified of the tank <br /> removal. <br /> 2. La Finca #1 Partnership will be responsible for the removal of the soil from the <br /> tank removal, if contaminated, pursuant to the letter from William J. Pendola,Jr. <br /> dated December 10, 1999. <br /> 3. The owner of the tank is La Finca #1 Partnership, not the U.S. Federal <br /> �.Government. <br /> 4. All inspections must be scheduled at least 48 hours in advance. <br /> 5. Call USA Dig prior to any excavation. <br /> 6. Underground Storage Tank Disposition Tracking Record must be submitted <br /> within 30 days of tank removal. <br /> 7. The Backfill Excavation Certificate must be submitted within 30 days of the tank <br /> removal with verification of backfill material. <br /> 8. The Tank Closure Certification form must be submitted within 30 days of tank <br /> removal. <br /> 9. Sample analysis must be conducted in accordance with the leaded gasoline <br /> specifications on page 8 of the permit application. <br /> 10.The amount of samples taken will be based on the specifications on page 7 of the <br /> permit application. <br /> 1 l.The sampling firm must be an independent third party from the contractor. <br /> 12.Analytical results must be submitted directly from the lab to PHS-EHD within 30 <br /> days of the tank removal. <br /> Please contact me at (209) 468-9848 to schedule inspections and with any questions. <br /> A Division of San Joaquin County Health Care Services <br />
