The dates for these <br />tests were listed as 5-5-14, These should have been listed as 5-15-15. During the monthly inspection, the designated <br />operator shall check that all required testing and maintenance for the UST system have been completed and <br />document the dates they were done. During the inspection, the designated operator corrected the dates for these <br />tests, Continue to ensure that designated operators performing monthly inspections at this facility are including all of <br />the required information on the reports. <br />Corrected on site. <br />The designated operator failed to document all the alarms from the attached alarm history on the designated operator <br />monthly inspection reports and failed to check that they were responded to appropriately. The missing alarms <br />Include: L1 diesel turbine sump sensor fuel alarms on 11-15-15, 11-10-15, and the sensor out and fuel alarm on <br />6-10-15. The designated operator documented the 11-2-15 fuel alarm but did not document the response to the alarm, <br />The designated operator did not document the response to the L2 diesel annular sensor low liquid alarms on 9-24-15 <br />and 8-30.15. The designated operator did not document the 8-29-15 low liquid alarm and the response to h. During the <br />monthly inspection, the designated operator shall review the alarm history for the previous month, check that each <br />alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation <br />taken in response to any alarms to the monthly report Ensure that designated operators performing monthly <br />inspections at this facility are including all of the required information on the reports. During the inspection, the facility <br />designated operator was reminded to correctly complete the monthly designated operator inspection reports. <br />This was corrected on site. <br />This Is a minor violation. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br />The diesel turbine sump sensor was removed from the lowest point of the sump and placed on its side at the highest <br />portion of the bottom of the sump and not located to detect a leak at the earliest opportunity. Monitoring equipment <br />shall be maintained to be able to detect a leak at the earliest possible opportunity. The terminal manager stated that <br />he did not know that the sensor was supposed to be located to detect a leak at the earliest opportunity. The sensor <br />was relocated to the lowest point of the sump by the service technician.
