This work was completed on June 10, 2015. The permit has <br />been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br />applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Contact Elena Manzo at (209)'953-7699 to arrange the following appointments (a 48-hour notice is <br />required for inspection appointments). Please be aware that inspections for emergency work must be <br />scheduled within five days of permit approval. <br />• Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br />monitoring has been restored to meet the requirements of Title 23 California Code of Regulations <br />and the Health & Safety Code. A copy of the alarm history shall be printed prior to performing <br />the "cold start" of the monitoring system and made available at the time of EHD inspection. <br />4. If concrete will be broken near the tank, a secondary containment test must be performed on the <br />tank once concrete is poured in accordance with the manufacturer's guidelines. <br />Any individual performing the work of a service technician must meet the following qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, current <br />contractor's licenses issued by the Contractors State License Board (may be issued to the <br />installation company), or a Tank Tester's license issued to the individual by the State Water <br />Board, and <br />b. Possess or work under the direct and personal supervision of an individual physically present at <br />the work site who possesses a valid, current California UST Service Technician certificate issued <br />by the International Code Council (ICC), and <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />
