Monitoring System Certification <br />test results for 2012 were not found on site. These records shall be maintained on site for at least three years. <br />Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br />maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br />field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) <br />results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring <br />records for the last three years on site and submit copies to the EHD. Facility contacted the service company and <br />obtained a copy of Monitoring System Certification test results for 2012 at the time of inspection. This was corrected <br />on site. <br />This is a minor violation. <br />208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />The February 2015 designated operator monthly inspection report was not found on site. Designated operator <br />monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of <br />the previous twelve months of designated operator monthly inspection reports are maintained on site. Facility <br />contacted the service company and obtained a copy of the February 2015 designated operator monthly inspection <br />report at the time of inspection. This was corrected on site. <br />This is a minor violation. <br />309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br />The Bravo box float and chain in the diesel UDC sump failed to stop the flow of product at the dispenser when tested. <br />All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br />dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. This was <br />corrected on site. <br />This is a minor violation. <br />328 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The diesel line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a <br />3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or <br />replace the failed leak detector (LG 113-x listed, if applicable) and obtain a permit within one business day from the <br />EHD. The technician retested the diesel line leak detector and it passed after the third attempt. This was corrected on <br />site. <br />This is a minor violation. <br />Overall Inspection Comments: <br />Notes: There were 4 violations noted during this inspection.
