The monitoring and response plans on site are not current and not approved by the EHD. The <br /> monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br /> of the monitoring and response plans for approval by the EHD within 30 days of receiving this report. An <br /> approved copy will be returned for your records. <br /> 201. Alarm log for alarm on 1/21/12 (1-1), 8/30/11 (1-2) and designated operator monthly inspection report <br /> for the month of April, 2012, were not found on site. These records shall be kept on site for at least 3 <br /> years. Immediately locate and maintain all alarms that occurred on site and submit copies to the EHD <br /> within 30 days of receiving this report. <br /> 205. The April, 2012, designated operator monthly inspection report were not found on site. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit April, 20012, D.O monthly report to the EHD within 30 days of receiving this <br /> report. <br /> 206. The designated operator employee training record was not found on site. The designated operator <br /> shall train facility employees for which he or she is responsible in the proper operation and maintenance <br /> of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
