Obtain an EPA ID number from the Department of Toxic Substance Control (800-61- <br /> TOXIC)prior beginning removal. <br /> 2. All analytical results must come from a California certified laboratory or this office <br /> will not accept them. <br /> 3. See pages 7 & 8 of the permit application for required analytical tests. (Leaded) <br /> 4. A copy of the fire department permit must be submitted at the time of removal. <br /> 5. Schedule removal appointment 48 hours in advance. <br /> 6. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> (a) Analytical results,mailed from the lab <br /> (b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br /> (c) Tank Closure Certification Form <br /> (d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt <br /> 7.
