1.0 INTRODUCTION <br />This Health and Safety Plan covers the activities to be carried out during the removal <br />of a STIP3 double wall steel tank and 10 ft of fiberglass pipe at 103 North East St., <br />Stockton, California. This plan may be upgraded/downgraded at any time, in light of <br />actual site conditions, at the discretion of the Site Health and Safety Officer. <br />This plan covers all contractor involved in the site activities. It serves as a minimum <br />guideline for protective measures. Individual contractors may elect to implement <br />more stringent measures for their own workers. Each contractor will provide health <br />and safety equipment for its employees only. All other personnel are expected to <br />provide equal or greater levels of protection for themselves. <br />All on-site personnel, regulatory agency personnel, and visitors are expected to be <br />familiar with, and comply with the provisions of this plan. <br />2.0 SITE BACKGROUND <br />The existing 1,000 STIP3 double walled steel tank was installed in 1988 and was <br />used for waste oil. The tank will be removed along with 10 ft. of fiberglass pipe. <br />3.0 KEY PERSONNEL AND RESPONSIBHATES <br />On -Site Excavation Contractor Supervisor — Mr. John Savidge, Savidge Consttvchon Inc. <br />The On -Site Excavation Contractor Supervisor is responsible for oversight of the <br />excavation, tank removal, and associated activities. He is directly responsible for <br />construction personnel compliance with the health and safety plan, and will act as Site <br />Safety Officer (SSO) for Savidge Construction personnel. He will also function as <br />the competent person for excavation purposes and as the emergency coordinator in <br />the event of an onsite emergency. <br />4.0 JOB HAZARD ANAYLSIS <br />4.1 Physical Hazards <br />The major physical hazards associated with the site are expected to include: <br />a. Potential heat stress <br />b. Work around construction equipment <br />C. Noise <br />d. Manual material handling/demolition <br />e. Open excavations <br />f. Tank and pipeline removal <br />g. Confined space entry <br />
