0 0 <br />October 7, 2016 <br />Stacy Rivera, <br />Senior REHS <br />San Joaquin County EHD <br />1868 Hazelton Avenue, <br />Stockton, CA 95205 <br />Dear Inspector Rivera, <br />Thank you for your visit to our location on August 18 & September 9, 2016. During your visit, you brought a few items to <br />aur attention. We have noted each issue below with our plan to address them. In addition, please seethe attached <br />documentation. <br />11 Alleged Violation (m): CCR 2712(6) Failed to maintain alarm logs and/or records of follow up actions: Maintenance <br />and monitoring records for the lost three years were not found on site. <br />Response: The site was compliant according to CCR 2712 (b) which states "written monitoring and maintenance <br />records shall be maintained on-site or off-site at a readily available location". These records shall be made <br />available, upon request within 36 hours, to the local agency. Our gas station and warehouse have one location <br />address: 2440 Daniels St. According to the manager Martin Turley and attendant Stacy Rodriguez who walked <br />with the inspector: the inspector did not request or ask for the records to be produced. <br />All records kept in the warehouse are considered "on site", and are currently up-to-date. <br />2) Alleged Violation (201)• CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions: <br />The missing records include responses to the following alarms: U (91 Annular) High Liquid alarm - 7/31/16 <br />Response: The Annular High Liquid Alarm from 7/31/16 was recorded on the Alarm Log. FMS responded to the <br />shutdown by dispatching Alltech. However; the alarm cleared by itself before Alltech could arrive on site that <br />Sunday morning. That detail was summarized as "FMS monitoring". <br />31 Alleged Violation (201): CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions: S11 (87A Vent <br />Vacuum) Communication alarm - 7/22/16 and S31 (87A Vent Vacuum) Communication alarm - 7/20/16 <br />Response: The Designated Operator (DO) (Belshire) noted both alarms 511 (87A Vent Vacuum) 7/22/16 and <br />7/20/16 Communication alarms on page 2 of the Designated Underground 5tgrage Tank (UST) Operator Monthly <br />Visual Inspection checklist dated 08/05/16 on page 2 of the checklist (see below). In addition, the DO stated the <br />alarm was an isolated event. This was documented and emailed to Gilbarco for confirmation and verification that <br />the sensors were working properly on August 8, 2016. Gilbarco responded August 8, 2016, confirming that <br />sensors were functioning properly that same day. <br />The 511 (87A Vent Vacuum) Communication Alarm on 7/22/16 was recorded on the Alarm History Report by the <br />Gas Station Attendant on 7/22/16. (See attached documentation Alarm History July 2016). This was also an <br />isolated event confirmed by Gilbarco. In addition, the alarm cleared within seconds and no documentation was <br />printed or spooled from the Veeder—Root on 7/20/16 for the Gas Station Attendant at the site (2440 Daniels St.) <br />to record. <br />All steps were followed according to CCF 2712(6); it was impossible to log alarm which cleared in seconds and no <br />spooling of paperwork. Again this was a unique alarm backed by verification from Gilbarco of alarm time and <br />length of alarm. DO complied to CCR 2712(b) with follow-up to verifying unit was functioning. <br />
