Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility- <br />- Corrected on site. Received copies of CFR and insurance policy statement. <br />23d. The designated operator employee training for employees are either not performed within the <br />twelve months or have not been performed. It is unclear whether employees Elnidino Arreola, Regina <br />Hurtado, Caryn Hall have been performed. No previous training records could be found. <br />Last training for Yvette Bobadilla, Rodney Olivera, Sara Harris, Irwin Garcinia, Jules Lavroette was <br />conducted on 0312412010. The designated operator shall train facility employees or which he or she is <br />responsible in the proper operation and maintenance of the UST system once every 12 months. The <br />training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employees's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employees's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a statement how to ensure all employees are trained once every twelve months. <br />47. The UDC 7f8 annular sensor failed when tested. The sensors monitoring the liquid level in the <br />interstitial space shall be maintained so that a breach in the primary or secondary containment is <br />detected before the hazardous substance is released into the environment. Correct immediately by <br />having a properly licensed, trained, and certified contractor replace the failed component with a <br />functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from <br />the EHD. <br />- Corrected on site. Sensor replaced on site.
