REHS <br />Re: Costco Wholesale #1031 SR0061144 <br />2440 Daniels St, Manteca, 95336 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to replace and install an OPW Impact (Shear) Valve on Dispenser <br />9110 on the underground storage tank (UST) system at the above noted address. The permit <br />has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Thuy Tran at (209) 468-8257 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Primary piping soap test: One hour pneumatic pressure test at 110% of operating <br />pressure, or 40 psi, whichever is greater. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State 'License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (IGC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />5. Qualified installers must possess or work under the direct and personal supervision of an <br />individual physically present at the work site who possesses all of the following: <br />a, A valid, current contractor's license issued by the Contractors State License Board, <br />and <br />
