<br />SUPERCEDES APPROVAL LETTER DATED FEBRUARY 14, 2005
<br />The San Joaquin County Environmental Health Department (EHD) has completed the review of the application
<br />for permit to install 3 new tanks at the above noted address. The permit has been approved with the following
<br />conditions:
<br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any
<br />applicable permits and requirements.
<br />2. Individuals installing UST Systems or components shall possess or work under the direct and personal
<br />supervision of an individual physically present at the work site who possesses all of the following:
<br />a. One of several permissible valid, current contractor's licenses issued by the Contractors State
<br />License Board (may be issued to the installation company), and
<br />b. A valid, current International Code Council (ICC) UST Installer/Retrofitting certificate and
<br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) being
<br />installed.
<br />3. Qualified service technicians shall possess or work under the direct and personal supervision of an
<br />individual physically present at the work site who possesses all of the following:
<br />a. One of several permissible valid, current contractor's licenses issued by the Contractors State
<br />License Board (may be issued to the installation company), and
<br />b. A valid, current California UST Service Technician certificate issued by the International Code
<br />Council (ICC), and
<br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) being tested.
<br />4. Each individual installing or testing any component shall hold a certificate of training from the
<br />manufacturer of that component.
<br />5. All piping, fittings, sumps, and under -dispenser containment must be certified by an independent testing
<br />organization and labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See
<br />California Code of Regulations, Title 23, Chapter 16 Table B for complete list.). Pursuant to the
<br />Underwriters Laboratories Inc (UL) listings, only those products bearing the UL mark will be considered
<br />listed (only applicable to UL listed products).
<br />6. All monitoring systems shall be listed on the LG113 (current version).
<br />7. All installation and testing shall be in compliance with manufacturer's instructions, section 2635 and
<br />section 2636 of CCR and Chapter 6.7 of the Health and Safety Code as amended and effective July 1,
<br />2003.
<br />8. All piping must slope and be able to drain to a containment sump with an audible/visual alarm.
<br />9. All tank top openings that are connected to the primary space of the UST must have secondary
<br />containment, audible/visual alarm and be vapor tight or terminate at grade.
<br />10. Sumps and under dispenser containment must be constructed in a manner to prevent vapors from
<br />entering the backfill.
<br />11. All secondary containment must be constructed to prevent water intrusion.
<br />12. All secondary containment systems must be continuously monitored as required under the California
<br />Health and Safety Code (HSC) 25290.1
