SITE MITIGATION: <br />a. In the event contamination is observed, confirmed or suspected as a result of a <br />leaking UST system, it is the responsibility of the owner or operator to submit <br />a work plan to EHD Site Mitigation Unit prior to initiating any assessment or <br />remediation activities. <br />b. To minimize delays, a Corrective Action Plan may be submitted concurrent <br />with tank closure or installation plans. Address one copy of the Corrective <br />Action Plan to the Site Mitigation Unit for review. For further assistance <br />contact the Site Mitigation Unit at (209) 468-3450. <br />B. PLAN REVIEW AND OPER kTING PERMIT FEES <br />1. New facility plan review fee of $784.00 (8 hour minimum per facility), $550.00 <br />facility fee (includes one tank), $125.00 per tank after the first tank, plus $15.00 <br />state surcharge fee per tank is due at time of plan submittal plus $24.00 per site <br />state surcharge. <br />2. New installation of an additional tank at an existing facility is $784.00 plan <br />review fee (8 hour minimum per facility) plus $125.00 per new tank after first <br />tank, and $15.00 per new tank state surcharge fee. <br />3. It should be noted that a $784.00 plan review check fee required for each <br />submittal allows for up to eight (8) hours of staff review and/or inspection time. <br />Additional time spent on plan review, meetings, inspections, phone or office <br />consultations will be billed to the applicant at a rate of $98.00 per hour. <br />4. The maximum review time for UST installation plans is 40 working days. A <br />preliminary review will be conducted by the EHD within 20 working days of <br />submittal date to identify any gross plan deficiencies. <br />5. If gross deficiencies are identified, the plans will be returned with a checklist <br />identifying plan deficiencies. <br />6. To expedite the review of installation plans, time and a half per hour overtime <br />fee may be submitted to have review completed within 10 working days of <br />submittal date if staff is available to perform the review. <br />Revised 8/3/07 2 <br />
