No. TBD f Our Job No. 12919 <br />Dear Kasey: <br />RD EcoVED <br />SES' % 7 2007 <br />EPaPERN1MsPRVICES <br />On behalf of our client, Costco Wholesale (Costco), Barghausen Consulting Engineers, Inc., is submitting <br />for an Underground Storage Tank Permit for a new fueling facility at the location referenced above. This <br />project will include construction of a fully automated, members -only, self -serve retail fueling facility. <br />The fueling facility will consist of a new 32- by 120 -foot pump island canopy with eight (8) multi -product <br />dispensers (16 fueling positions). The project also includes the installation of three (3) 30,000 -gallon <br />underground gasoline storage tanks and an S- by 14- foot controller enclosure that will house the facility's <br />electronic monitoring .and alarm system. <br />The following items are enclosed for your review: <br />One (1) completed New UST Application, including fee sheet - The contractor information <br />will be provided at the time of permit issuance. <br />2. One (1) UST Service Request Form <br />3. One (1) UST — Facility form (Form A) <br />4. Three (3) sets of UST — Tank pages 1 and 2 (form B), one for each tank <br />5. Three (3) sets of tank and piping plans <br />6. One (1) set of manufacturers' equipment cut sheets <br />7.
