WESTERN METER SERVICE, INC. <br /> HEALTH and SAFETY PLAN <br /> Kingdon Air Park <br /> The site location for the proposed job of removing one (1) 10,000 gallon <br /> aviation fuel tank as follows: <br /> Kingdon Air Park <br /> 12145 N. Devries Road <br /> Lodi, CA 95242 <br /> The project manager, and the health and safety coordinator will be Burl <br /> DuBose of Western Meter Service, Inc. , 2735 Teepee Drive, Suite E, Stockton, <br /> Ca 95205, (209) 948-6124. The site safety officers at the job site will be <br /> Burl DuBose and/or Weslev DuBose of Western Meter Service. The alternate site <br /> safety officer will be John Swets of Western Meter Service. <br /> Western Meter Service will have on the site one (1) fire extinguisher <br /> and one (1) first-aid kit within fifty (50) feet of the tank or areas where <br /> flammable vapors are present . <br /> Western Meter Service, Inc. shall notify the local Fire Department 48 <br /> hours before excavationshall begin. USA will also be notified before excava- <br /> tion of job. <br /> The tank must be empty of product before start of job. Western Meter <br /> Service will triple rinse the tank with soap and water. Rinse will be pumped <br /> from the tank and hauled away by Refineries Service, Inc. , Patterson, Ca <br /> 95363, (209) 892.6742, a waste hazardous hauler, for disposal. <br /> The tank will be dry iced, 15 pounds of dry ice per 1,000 gallons, <br /> before excavating. Western Meter Service shall monitor tank with a Combus- <br /> tible Gas Indicator (CGI) and seal all openings before tank will be <br /> transported. At no time will Western Meter Service cut or weld on the tank. <br /> The tank will be secured on truck after extraction for transportation. <br /> The tank and plumbing will be hauled by Redfearn Trucking, E. Mariposa Road, <br /> Stockton, Ca, to 'Triangle Inc. of Sacramento, 3525 52nd Avenue, Sacramento, Ca <br /> 95823 to be destroved. All of the above procedures will be done according to <br /> code. <br /> Western Meter Service will approach all tank excavation with possible <br /> cave-ins. The following rules will be followed to prevent any mishaps: 1) <br /> soil to be inspected for stability before and after tank is removed, 2) soil <br /> is to be kept at least two (2) feet from the edge of an excavation, 3) if ex- <br /> cavation is five (5) feet or more the hole shall be tapered, braced, or other <br /> equivalent methods shall be used to prevent possible cave-ins.
