According to the training list on site, the designated operator employee training for Rajinder Kumar, <br /> Maria Valdez, and Seema Narula was not current. Mr. Kumar and Ms. Valdez were the only employees <br /> on site upon arrival for the inspection. The designated operator shall train facility employees for which <br /> he or she is responsible in the proper operation and maintenance of the UST system once every 12 <br /> months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> A list of facility employees who have been trained by the designated operator shall be maintained on <br /> site. Ensure that employees have been trained by the designated operator, maintain the list on site. A <br /> training list showing current training for the above employees was faxed to the facility at the time of <br /> inspection. <br /> 302, 304 & 305. The alarm light on the monitoring panel was not functional at the time of inspection, <br /> therefore, the annular and sump sensors failed to activate a visual alarm when tested. The interstitial <br /> space shall be monitored with a monitoring system connected to an audible and visual alarm system. All <br /> monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. The light was replaced at the time of inspection. <br /> 302 & 308. The test boot in the 91 STP sump was not pulled back. The Schrader valve was open, but <br /> was pointing out to the side, instead of down. Any potential release from the primary containment into <br /> the secondary containment in the 91 octane piping does not flow into a collection sump. All secondary <br /> containment systems shall be sloped so that all releases will flow to a collection sump located at the low <br /> point of the underground piping.
