This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The Site Plan indicates that there is a diesel UST on site which is incorrect. The diesel UST was converted to an 87 UST. A site plan must be submitted identifying the locations where monitoring will be performed. This is a minor violation. 123 CCR 2661 Permit to repair UST system not obtained. The mechanical flapper for the 87A(south) UST was replaced on July 23, 2015. An approval letter for the repair permit was issued by EHD on August 17, 2015. The approval letter required that the inspection for the overfill prevention verification be scheduled within five days of permit approval and stated that the facility cannot receive fuel in the tank prior to the EHD inspection and approval. The facility has not ceased fuel deliveries into the 87 tank. The overfill prevention verification inspection was conducted today (10/23/15), two months past due. The All conditions included on the UST repair approval letters issued by EHD shall be followed at all times. Ensure that all conditions from UST repair approval letters in the future are followed and all inspections are performed within the required time frames. This is a Class II violation. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. The maintenance record for the installation of the 87 UST flapper completed on July 23, 2015,was not found at the facility. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. This was corrected during the inspection when the service technician from Elite IV provided the maintenance record to the facility. This is a minor violation.
