This work was completed on July 23, 2015. The <br /> permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Overfill prevention verification: Measurement of the drop tube to verify the 95% <br /> tank volume shut off. Ensure that all pertinent tank charts and calculations are <br /> available for review at the time of inspection. <br /> Please be aware that the facility cannot receive fuel in the tank prior to the EHD <br /> inspection and approval. <br /> 4. If concrete will be broken near the tank, a secondary containment test must be <br /> performed on the tank once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
