Maintenance records on site indicate a cold start of the monitoring panel was performed on March 30, 2014 and a permit was not obtained from the EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit from the EHD for the monitoring panel cold start. A"Failure to Obtain a Permit" penalty applies. This is a Class II violation. 204 Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not found on site. A copy of these plans shall be retained on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class II. 206 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment testing on the July through December 2013 and the January through April 2014 designated operator monthly inspection report. Secondary containment testing was last conducted on March 19, 2012. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by June 22, 2014. Consent to perform the inspection, take photos and collect samples was given by the owner, Muhammad (Johnny) Rizwan. This facility's designated operator is Dan Mcllrath (ICC expiration date: 5/31/14) . The service technician was David Winkler (ICC expiration date: 3/3/16), who had the following manufacturer's certifications: Veeder-Root Level 4 (expiration date: 2/10/16) VMI Tester (expiration date: 11/10/15).
