The tank forms do not list the type of the <br /> corrosion protection. Any change of information must be updated in CERS within 30 days of the changes. <br /> Immediately log into CERS, update the required information, and submit for review by the EHD. <br /> This is a Class II violation. <br /> 125^ CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on May 29, 2015 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by June 28, 2015. This <br /> information has not been received resulting in a non-compliant status for this facility. An operator that receives an <br /> inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Open violations not <br /> corrected: <br /> -CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> -HSC 2540 (e)(4) Failed to report program data electronically. <br /> This is a Class II violation. <br /> 126 HSC 25404(e)(4) Failed to report program data electronically. <br /> The designated operator form was not submitted to the the California Electronic Reporting System (CERS). Beginning <br /> January 1, 2014, all businesses are required to submit all new(or any changes to existing) UST information, including: <br /> UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator <br /> Written Agreement (if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST <br /> Operator Compliance online to the CERS at http://cers.calepa.ca.gov. Be sure to include any other relevant activities <br /> and required fields. <br /> This is a repeat violation, Class II. <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 91 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br /> a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. This <br /> was corrected on site. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by June 23, 2016 . <br /> Page 4 of 5 <br />
