A site plan must be submitted identifying the locations <br /> where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> This is a minor violation. <br /> 126 HSC 25404(e)(4) Failed to report program data electronically. <br /> The UST Response Plan and Owner Statement of Designated UST Operator Compliance were not submitted to the <br /> the California Electronic Reporting System(CERS). Beginning January 1, 2014, all businesses are required to submit <br /> all new(or any changes to existing) UST information, including: UST Monitoring Site Plan, UST Certification of <br /> Financial Responsibility, UST Response Plan, UST Owner/Operator Written Agreement(if applicable), UST Letter from <br /> Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance online to the CERS at <br /> http://cers.calepa.ca.gov. Immediately upload the required forms to the CERS website. Be sure to include any other <br /> relevant activities and required fields. <br /> This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to check and note the correct test dates for the secondary containment(SB989)testing <br /> on eleven of the designated operator monthly inspection reports in the last year. It was listed as: <br /> -3/19/14 on the April and May 2015 reports <br /> -3/20/13 on the September, October, November, and December 2014 and January and February 2015 reports <br /> -3/2013 on the July and August 2014 reports <br /> -3/2012 on the June 2014 report <br /> Secondary containment testing was conducted on March 19, 2012 and March 18, 2015. During the monthly <br /> inspection, the designated operator shall check that all required testing and maintenance for the UST system have <br /> been completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 91 VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting <br /> a 3-gallon per hour leak at 10 psi.
