CASE SUMMARY <br /> On January 28, 2004, Ms. Dina Abate, Registered Environmental Health Specialist <br /> (REHS), San Joaquin County Environmental Health Department (EHD), performed an <br /> underground storage tank (UST) consultation with Mr. Muhammad Rizwan of SRH Food <br /> and Gas. Mr. Rizwan was the new tank and property owner and was given the following: <br /> a monitoring plan form, a response plan form, an UST —Facility form (former Form A), <br /> and a certification of financial responsibility form. Ms. Abate explained to Mr. Rizwan <br /> all of the provided forms must be completed and submitted to this office now that he is <br /> the owner of the UST facility. Ms. Abate informed Mr. Rizwan about annual UST <br /> requirements and a 6-month secondary containment test would be required because new <br /> under dispenser containment(UDC) sumps were recently installed. Mr. Rizwan was also <br /> informed that the facility's turbine sumps failed secondary containment testing and would <br /> have to be repaired. Mr. Rizwan submitted the completed UST — Facility form to the <br /> EHD accounting office prior to leaving the EHD (Attachment 1). <br /> On June 11, 2004, Ms. Abate conducted a routine UST inspection (Attachment 2). <br /> During the routine inspection, Ms. Abate asked to see the monitoring plan, response plan <br /> and certification of financial responsibility. None of the paper work requested was <br /> located at the facility. Ms. Abate provided copies of these forms to the facility. The <br /> inspection report left at the facility stated 6-month secondary containment testing was <br /> overdue (required by February 20, 2004) for the under dispenser containment. The <br /> facility was instructed to conduct 6-month secondary containment testing immediately. <br /> Ms. Abate also required a permit be obtained from EHD immediately to repair turbine <br /> sumps that failed secondary containment testing on March 25, 2003.
