The facility still did not have an EPA ID number, but it was not noted <br /> in the inspection report. <br /> On August 24, 2005, Ms. Le sent follow up letters (Attachment 20) certified to the <br /> facility and Mr. Rizwan's home mailing address asking for items noted in the July 13, <br /> 2005, UST inspection report. Financial responsibility documents were received on <br /> August 16, 2005,but employee training records were not submitted. <br /> On March 27, 2006, secondary containment testing was performed by R&D Compliance <br /> Testing. All components, including tank top sumps, were tested and passed. A report <br /> was received by this office on April 10, 2006(Attachment 21). <br /> On May 16, 2006, Mr. Muniappa Naidu, Sr. REHS, EHD, performed a routine UST <br /> inspection (Attachment 22) and witnessed the annual monitoring system certification <br /> (Attachment 23) and found that the monitoring and response plans were not current or on <br /> site. These were later submitted on June 15, 2006. <br /> On October 19, 2006, Mr. Ray von Flue, Sr. REHS, EHD, went to the facility to inspect <br /> an annular sensor that had been replaced under a permit (Attachment 24). Mr. von Fliie <br /> asked the technician open up a turbine sump and found that the secondary containment <br /> failures from the penetrations had been fixed using a split boot. <br /> On May 24, 2007, Mr. von Flue performed a routine UST inspection (Attachment 25) <br /> and witnessed the annual monitoring system certification (Attachment 26) and found that <br /> current financial responsibility documents had not been submitted to this office. These <br /> were later submitted on June 21, 2007. <br /> Attachments 27, 28, and 29 include property owner information and facility file <br /> narratives for the UST and hazardous waste files, respectively. <br /> II.
