This <br />information was required to be submitted by February 16, 2009. Part of this information was submitted on March <br />19, 2009, however some items are still missing. The Designated Operator for this facility was changed with out <br />notifying our department within thirty days. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($105).
