EI Dorado St., Stockton CA <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to remove the underground storage tank system (UST) at the above <br /> noted address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. See pages 7 & 8 of the permit application for the required number of samples and <br /> analytical tests to be performed. Form EH 23 046 (Revised 7/22/10) must be utilized. <br /> 4. If the backfill material and soil are found to be contaminated it must be disposed of at an <br /> approved disposal location. Treatment of soil on site can only be performed under a <br /> hazardous waste treatment permit. <br /> 5. The excavation must be secured if it will not be backfilled immediately. <br /> 6. A copy of the "Site Health & Safety Plan" must be on the job site available for review. <br /> 7. A copy of the Fre department permit must be submitted at the time of the removal. <br /> 8. The following information must be submitted to the EHD within thirty days after the UST <br /> removal date: <br /> a. Only analytical test results that come directly from a third party California certified <br /> laboratory with an affixed chain of custody will be accepted by the EHD. <br /> b. The UST Tracking Record Sheet(s)and/or Tank Hazardous Waste Manifest(s). <br /> c. Tank Closure Certification Form (DTSC Form 1249). <br /> d. Hazardous Waste Manifest or disposal receipt for piping & other related <br /> appurtenances, waste liquid, residual fuel, waste oil, etc. <br />
