El Dorado, Stockton <br /> The permit application to remove an underground storage tanks from the above <br /> referenced facility has been approved with the following conditions: <br /> 1. If the backfill material and soil is found to be contaminated it must be disposed of at <br /> an approved disposal location. Treatment of soil onsite can only be performed under a <br /> hazardous waste treatment permit. <br /> 2. See pages 7 & 8 of the permit application for required analytical tests. <br /> 3. A copy of the fire department permit must be submitted at the time of removal. <br /> 4. Schedule removal appointment 48 hours in advance. <br /> 5. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> (a) Analytical results,mailed from the lab <br /> (b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br /> (c) Tank Closure Certification Form <br /> (d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt <br /> (e) Submit the backfill excavation certificate as required by San Joaquin County <br /> If you have any questions please contact Dennis Catanyag at(209) 468-3459. <br /> Thank you, <br /> Dennis Catanyag, .E.H.S. <br /> Registered Environmental Health Specialist <br />
