Sep -02-99 01:07P Evan= 1ruthevs Inc. 925 3 0229 P.17 <br />Gateway Project - Stockton CA Evans Brothers Inc. <br />non -sudden release or hazardous waste or hazardous waste constituents to air, <br />soil, or surface water at the facility, the team member that observes this <br />condition shall give an emergency alarm. <br />Actions taken will be dictated by the emergency. All appropriate local emergency <br />response agencies shall be notified immediately. The police, fire department, <br />emergency response teams and ambulance may be reached via telephone by dialing 911 <br />from a cellular phone. <br />The nearest hospital and additional emergency contacts are listed above. <br />Personnel encountering a hazardous situation shall instruct others on site to evacuate the <br />vicinity immediately and call the (1) Site Safety Manager, (2) the Project Manager, and <br />(3) the Health & Safety Hygienist for instructions. <br />The site must not be re-entered until back-up help, monitoring equipment, and personal <br />protective equipment are on hand. <br />Should an off-site hazardous spill occur, the safety and emergency procedures will be <br />governed by those of the carver, hauler, ctc. <br />Occupational Safety and Health Guidance Manual for Hazardous Waste site <br />Activities; Prepared by NIOSH, OSHA, USCG, EPA <br />9.3 Usual Procedures for Injury <br />I) If the injury is minor, proceed to administer first aid. <br />2) Notify the Site Safety Manager and Project Manager of all accidents. <br />3) If the injury requires medical attention, notify the Site Manager and Project <br />Manager. <br />4) Telephone for ambulance/medical assistance if necessary_ Whenever possible, <br />notify the receiving hospital of the nature of physical injury or chemical <br />overexposure. If no phone is available, transport the person to the nearest hospital. <br />5) Send/take this SSP with the MSDSs to the medical facility with injured person_ <br />Worker Compensation Insurance Information Packets are in the site office. <br />6) Notify the Site Safety Manager and Project Manager of all accidents, incidents and <br />near -miss situations. <br />7) Complete Accident/Incident/Near-Miss Form as needed. <br />8) Notify and provide necessary information to the engineer for all accidents, <br />incidents, and near -miss situations. <br />9.4 Emergency Treatment <br />When transporting an injured person to a hospital, bring this Site Safety Plan to assist <br />medical personnel with diagnosis and treatment. In all cases of chemical overexposure, <br />follow standard procedures as outlined below for poison management, first aid, and, if <br />applicable, cardiopulmonary resuscitation.
