The 91 and 87-master annular sensors were sifting in the dirt, not in the interstitial space upon arrival. The interstitial space shall be monitored with a monitoring system connected to an audible and visual alarm system. According to the alarm history there had not been any alarms since the last monitoring certification 2/2014. Secondary containment testing of the system was perfromed July 2014. No other maintenance documents were seen. The technician onsite tested the sensors during the monitoring certification. Both activated the audible and visual alarms. After testing the sensors were placed back into their proper location. Take all necessary precautions in the future to ensure the interstitial space is monitored. Corrected on site. This is a Class II violation.
