Implementing Agencies -2- <br /> Application for Amendment-An application for amendment is required <br /> wnenever: <br /> 1. A person installs or replaces a tank at a facility for which a <br /> valid operating permit has been issued for the entire facility; i .e. , <br /> files an application for the new or replacement tank. <br /> 2. A person with a valid operating permit stores a hazardous substance which <br /> is not listed in his application and seeks an amendment to his permit <br /> [H and S Code, Section 25286(d)]. This does not include a change from one <br /> type of motor vehicle fuel to another. <br /> Actions not requiring application for an amendment or new permit and conse- <br /> quently not requiring collection of the surcharge include: <br /> 1. • An unauthorized release. <br /> 2. Permanent closure or removal of a tank. <br /> 3. Change in monitoring procedure. <br /> 4. Repair of a tank. <br /> 5. Minor changes or corrections in the information on the permit file <br /> (e.g. , name change, change in contact person). <br /> 6.
