The <br /> report will be compared with the previous days results to identify <br /> any ongoing trends or differences that would indicate product <br /> leakage. <br /> All visual inspection records and tank monitoring printouts will <br /> be filed and retained for a period of three years. All recordable <br /> leaks and spills will be recorded on a Hazardous Material Incident <br /> Report, Form Number FR-0023. This form is also illustrated in <br /> Appendix III . <br /> F. EMERGENCY EQUIPMENT <br /> A number of registered fire extinguishers are kept on the premises <br /> at all times. The location of the emergency equipment is shown on <br /> the Vicinity Map and the Facility Storage Map presented in <br /> Appendix IV. Additional emergency equipment will be obtained as <br /> needed from Pacific Bell contracted hazardous material and <br /> hazardous waste handling vendors. <br /> The fire extinguishers are maintained and serviced by a <br /> licensed Pac Bel (Building Technician) on a yearly basis . <br /> A visual inspection is made by the facility Technicians on <br /> a routine basis to ensure the adequacy of the emergency equipment. <br /> G. ADDITIONAL INFORMATION (Contingency Plan) <br /> Unauthorized releases (leaks and spills) of petroleum products <br /> will be recorded utilizing the Hazardous Material Incident Report, <br /> Form No. FR-0023 and reported by the operator of the underground <br /> storage tank immediately to' the following : <br /> 1) Pacific Bell Emergency Control Center <br /> (415) 823-7777 <br /> 2) Real Estate Department <br /> (415) 823-1028 <br /> 3) Respective upper management within group. <br /> 4) Headquarters Staff <br /> In instances involving spills, the operator of the underground <br /> tank will notify the San Joaquin Local Health Department in <br /> Stockton at (209) 466-6781 as required by the County Ordinance. <br /> In case of fires, the local fire department (911) and Pacific Bell <br /> Emergency Control Center will be notified promptly by the operator <br /> of the underground tank. <br /> -4- <br />
