Box 388 <br /> Stockton,CA 95201-0388 <br /> Re: Tank Removal: Addendum <br /> 16865 Gawne Road <br /> Dear Ms.Briggs: <br /> In reference to the tank closure permit which you received on October I7, 1994, <br /> Geological Technics Inc. submits the following information. <br /> 1) Enclosed is the applicant signature (from Pg. 3, T. C. PIan) <br /> 2) The location of the septic system is unknown at this point, however, it is <br /> likely to be on the northern side of the house, in one of the yard areas (see <br /> attached) This supposition is based on the fact that the south, east and west <br /> sides of the house are paved with asphalt, thus it would be unlikely that the <br /> septic tank would be located in this area (See Enclosed). <br /> 3) Mr. Light's license is currently under yearly review at the State Contractors <br /> Licensing Board (800) 321-2752. As of a phone check 11/04/94, the license is <br /> current and active. <br /> 4) The tank will be triple rinsed and the rinsate hauled by Enviropur West of <br /> Patterson (a petroleum recycling company-hauler reg.# 2591). Mr. Light will <br /> haul the tank to A & S Metals in Modesto <br /> 5) The tank and piping disposal will be hauled together to A & S Metals. A copy <br /> of the receipt from the facility and the tank tracking record will be forwarded <br /> to P14S-SJC-EHD- <br /> 6) A dated authorization sheet to release analytical results is enclosed <br /> 6) With regard to third party sampling, Geological Technics Inc. personnel will <br /> be on site during the performance of the actual tank removal. <br /> 7) The Backfill Excavation Certificate will be submitted with the tank tracking <br /> record upon completion of the job. <br />
