The Closure Plan requires the following items: <br /> 1. Applicant signature on closure application. <br /> 2. The location of septic tank system on plot plan. <br /> 3. Current contractors license information for Del-Tech has been supplied. The <br /> license is current on 11/4/94. <br /> 4. Tank and piping disposal shall be performed in one of the following ways: (a) <br /> triple rinse tank and haul rinsate by licensed hazardous waste hauler or (b) haul <br /> tank under manifest by a licensed hazardous waste hauler to an approved disposal <br /> facility. <br /> 5. Piping is to be hauled with the tank, and proper disposal documented on the <br /> tank tracking record. <br /> 6. Dated authorization to release analytical results. <br /> 7. Submittal of Backfill Excavation Certificate within 30 days of tank removal (see <br /> attached certificate). <br /> 8. Update the Health and Safety Plan with information on the hospital to be used in <br /> case of emergency. Provide a map and highlight the travel route to local hospital. <br /> 9. State surcharges of$112 are due on this unregistered storage tank and must be <br /> paid before tank removal can occur.
