The San Joaquin Local Health District, Division <br /> of Environmental Health was designated as the enforcement .agency fur the <br /> cities and unincorporated areas within San Joaquin County. State law provides <br /> for a fee system to cover the cost of implementing this state mandated <br /> program. <br /> Local fees (see attached Fee Schedule), for these required inspectional <br /> services, will be billed on a yearly basis. The facilities Permit to operate <br /> will be issued for a five year period. In addition to the yearly <br /> inspectional fee, this statement will also include 3 $56/tank Sts to surcharge <br /> fee; The State surcharGe fee per tank will be rharaed every five year= n <br /> whenever the facilities permit is renewed or amended. All State surcharge <br /> fees will ,be transmitted by the Health District to the California Water <br /> Resourcess. Controi Board. <br /> To apply for a facilities Permit to Operate, complete the attached Fee <br /> Worksheet and submit the appropriate fees. Fees are due and payable February <br /> 3, 1986. Fees received after March 6, 1986 are subject to 100; penalty. <br /> in order to answer any questions regarding this Underground Storage Tank <br /> the San Joaquin Local Health Disti.ct Division of Envir_ .menta.l -!elith <br /> has scheduled the following information workshops: <br /> 1.
