All analytical results must come directly from a third party California certified <br /> laboratory with affixed chain of custody or this office will not accept them. <br /> 2. All hazardous waste must be treated, stored or disposed of at a permitted facility. <br /> © See pages 7 & 8 of the permit application for required analytical tests. <br /> '4. A copy of the fire department permit must be submitted at the time of removal. <br /> '-5. Schedule removal appointment 48 hours in advance. <br /> Y. In addition, the following information must be submitted within 30 days of the <br /> removal date: <br /> -(a) Analytical results,mailed from the lab <br /> -0) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br /> 4c) Tank Closure Certification Form(DTSC form 1249) <br /> —(d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt <br /> 7.
