Per the request in your e-mail, we will allow an extension to August 1, 2004 to submit the <br /> following additional information still needed to determine your eligibility for placement on the Priority <br /> List: <br /> • Claimant has indicated that corrective action has been completed. Please submit a copy of closure <br /> letter issued by the local regulatory agency. <br /> • Submit a copy of the closure report by the local agency. <br /> • Submit a removal permit for all underground storage tanks listed in claim application. <br /> • A copy of the permit to own or operate the UST from the local implementing agency dated between <br /> January 1, 1984 and January 1, 1990 (pursuant to Chapter 6.7 of the Health and Safety Code). <br /> If you were not subject to the permit requirement, submit documentation to confirm this claim. Situations <br /> where the permit was not required by January 1, 1990, can include: a) you removed all USTs prior to <br /> January 1, I990 and not replaced; b)you decommissioned all USTs pursuant to the direction of the <br /> regulatory agency prior to January 1, 1984; c)you sold the property and tanks by January 1, 1990. <br /> • On page two (2)of your claim application,you indicate that one 1,000 gallon diesel fuel tank was <br /> removed on 411988 and not replaced. However,you have submitted an operating permit for a 1,500 <br /> gallon motor vehicle fuel tank dated 615/92-12/31/92. Additionally, the map submitted shows the <br /> location of a replacement UST(on what date was the replacement UST installed?). Please clarify <br /> and/or revise claim application appropriately, and submit copy of latest operating permit for the 1,500 <br /> gallon tank(if it was removed, submit copy of removal permit/closure report by local agency). <br /> NOTE: Failure to respond to this request by August 1, 2004 may result in an ineligibility determination <br /> of your claim. <br /> California En vir0nment21Pr0tection Agency <br /> Recycled Paper <br />
