FRONTAGE RD RIPON CA 95366 <br /> A recent review of the underground storage tank file for the above referenced facility indicated this office has <br /> not received this years testing and paperwork requirements. <br /> The following information is overdue: <br /> CCR 2711(b)&(c)Current State Forms A&B <br /> HSC 25292.2 Certification of financial responsibility and letter from chief financial officer <br /> _CCR 2637(b)Annual monitoring certification <br /> _HSC 25284.2 Annual spill bucket/containrnent test <br /> _CCR 2630(d)Annual line leak detector test <br /> Most forms may be accessed from our web site address, www.zeocitias-condunitiii <br /> In addition,this office must be notified of any test date and time within 48 hours of performing the test. <br /> Furthermore,results from the testing must be sent to this office within 30 days of the test completion. Under <br /> CCR 2637(b)(4)it is the owner/operators responsibility to submit test results to this office.
