The following is an itemized list of hazardous materials violations that have not been <br /> addressed for VALLEY LUMBER as of May31, 2018. <br /> Open violations from May 18, 2017 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> The Hazardous Materials Inventory form or propane identifies the amount of propane as 400 pounds vs. 56 gallons <br /> that was observed at the time of inspection. This information must be current and submitted as part of the business <br /> plan. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The site map does not show the evacuation assembly area and emergency response equipment. A site map shall <br /> contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit <br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and <br /> emergency response equipment. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #8 - Failed to submit a training program or program is not reasonable or adequate for facility. <br /> The training program does not address annual training, including refresher courses, does not include safety <br /> procedures for the event of a release or threatened release of a hazardous material, including, but not limited to, <br /> familiarity with the facility's emergency response plans and procedures. These training programs may take into <br /> consideration of each employee. Immediately prepare or update the facility's training program, log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually for 2015, 2016 and 2017. The hazardous materials inventory shall be submitted by <br /> January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately <br /> log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for <br /> approval. <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. A site <br /> map was not completed or submitted as part of the business plan. <br /> Page 1 of 2 <br />
