Results of Testing/Servicing
Complete the following checklist:
Software Version Installed: 333.04
rYes ❑ No* Is the audible alarm operational?
❑ No* Is thevisual alarmo erational?
❑ No* Is the rint function o erational?
❑ No* Were all sensors visual) ins ec , functional) tested, and confirmed o erational?
❑ No* Were all sensors installed at lowest point of secondary containment and positioned so thatother
e ui ment will not interfere with their ro er o eration?
❑ No* If alarms are relayed to a remote monitoring station, is all communications equipment (e.g.
❑ N/A modem) operational?
NYes ❑ No* For pressurized piping systems, does the turbine automatically shut down if the piping secondary
❑ N/A containment monitoring system detects a leak, fails to operate, or is electrically disconnected?
If
yes: which sensors initiate positive shutdown? (Check all that apply) N Sump Sensors;
Dispenser Containment Sensors.
Did you confirm positive shutdown due to leaks and sensor failure/disconnection? N Yes; ❑ No.
NYes ❑ No* For tank systems that utilize the monitoring system as the primary tank overfill warning device, is
LFN/A the overfill warning alarm visible andaudible at the tank fill point(s) and operating properly? At
what ercent of tank ca act does the alarm tri er? 90%
Was any monitoring equipment replaced? If yes, identify specific sensors, probes, or other
equipment replaced and list the manufacturer name and model for all replacement parts in Section
E, below.
NWas liquid found inside any secondary containment systems designed as dry systems? (Check all
that appl )❑ Product; N Water. If yes, describe causes in Section E, below.
NWas monitoring system set-up reviewed to ensure proper settings? Attach set up reports, if
a licable
NIs all monitoring equipment operational per manufacturer's specifications?
1. Comments: There was a small amount of liquid found in the 87b fill sump from a recent additive leak
that was repaired but not fully cleaned up.
There was also a small amount of brine at the bottom of the additive Fill/STP. Inspected inside of sump and
found no visible leaks.
The s3 Additive vent sump sensor was going in to sensor fault. Cut the seal pack off and reconnected
VCWliquid float wires and sensor fault alarm never returned. It was either a bad section of the wiring or a bad
connection between the wires within the seal pack.
RECEIVE®
MAR 13 2017
ENVIRONMENTAL HEALTH
DEPARTMENT
