Contact the fire department and the San Joaquin ` alley Air(pollution Control District for <br /> applicable permits. <br /> 2. The contractor shall be licensed by the Contractors Licensing Board. <br /> 3. At least one person who holds the International Code Council (ICC) UST Installer/Retro.litter <br /> certification must be onsite when work is performed. <br /> 4. Any changes shall be submitted to El ID for approval prior to commencing work. All new <br /> submittals superccdc the previous ones. <br /> 5. All piping, fittings, sumps, under-dispenser containment and tanks must be ccrtified by an <br /> independent testing organization and labeled as required by the, organization. (tit_, UI_E, ANSI, <br /> Aipi, ASTM ect...See Title 23 Chapter 16 'Fable 13 for complete list) <br /> 6. All monitoring systems shall be listed on the LCI 13-16. <br /> 7. All testing shall be in compliance with manufacturer's instructions, section 2635 and section 2636 <br /> of California Code of Regulations (CCR) <br /> 8. All piping must slope to a containment sump with an audible/visual alarm. <br /> 9. All connections (spreader valves) from tike vacuum on the piping to the vacuum source must be <br /> turned down in the piping skimp. <br /> 10. All tank top openings, that are connected to the primary space ofthc UST must have secondary <br /> containment, audible/visual alarm and be vapor tight or terminate at grade. <br /> 11. Sumps and under dispenser containment must be constructed in a manner to prevent vapors from <br /> entering the backfill. <br /> 12. All secondary containment must be constructed to prevent water intrusion. <br /> 13. 'hhe secondary containment system must be continuously monitored as required under Califorhhia. <br /> I ealth & Safety Code (1-1 SC) 25290.1. <br /> 14. Overfill prevention shall satisfy CCR 2.635(b)(2)(B). <br /> 15. Underground Storage tanks are to be anchorc d per the permit application. <br /> 16. ]'lie tank owner must submit the owner statement and name/ICC certification information for this <br /> facility's, "California UST System Operator" (Designated Operator) prior to the fin�jl inspection. <br /> 17. A "Ccrtificate of Compliance for Underground Storage Tank Installation Forum C" shall be <br /> submitted by the owner or the owners' agent (pursuant to Califohila Code of Regulation (CCR) <br /> 2635(d)l before this upgrade will be considered complete by SJCEIID. <br /> 18.
