CONDITIONS <br /> 1 ) All samples must be taken by a representative of a state <br /> certified lab. <br /> 2 ) All solids, residuals, sludges, etc. that cannot be reused as <br /> a motor vehicle fuel are to be removed, handled, transported, <br /> disposed of as hazardous waste . A copy of the hazardous waste <br /> manifest is to be submitted to this office within 30 days of tank <br /> removal . <br /> 3 ) Submit completed tank tracking sheet within 30 days . of tank <br /> removal . <br /> 9 ) Obtain all necessary fire permits (with proof of permit on <br /> site at time of removal ) prior to scheduling appointment with <br /> S .J .L.H.
