The following are Repeat Class II <br />violations: 310, and the following are Minor -Notice to Comply violations: 201,204,206 <br />201. Document of owner response to 91 -octane sump (1-2) fuel alarm on 4/13/2012 by removing <br />rainwater as stated by the cashier during this inspection was not found on site. All maintenance records <br />shall be kept on site for at least 3 years. Immediately locate and maintain the on site and submit copies <br />to the EHD by 2/16/2013. <br />204. The designated operator failed to document the above-mentioned 91 -octane sump (1-2) fuel alarm <br />on 4/13/2012 on the April 2012 designated operator monthly inspection report and failed to check that <br />they were responded to appropriately. During the monthly inspection, the designated operator shall <br />review the alarm history for the previous month, check that each alarm was documented and responded <br />to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />206. Based on interview with Denny (cashier) during this inspection, Denny is not familiar with <br />emergency response and spill control procedure as required in the designated operator employee <br />training. The designated operator shall train facility employees for which he or she is responsible in the <br />proper operation and maintenance of the UST system once every 12 months. The training shall include, <br />but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by 2/16/2013. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />
