Harlan Rd., Lathrop CA <br /> The permit application to remove a 10,000-gallon underground storage tank (039- <br /> 0158701),piping and dispenser from the above referenced facility has been approved <br /> with the following conditions: <br /> 1. See pages 7 & 8 of the permit application for required analytical tests. <br /> 2. A copy of the fire department permit must be submitted at the time of removal. <br /> 3. Schedule removal appointment 48 hours in advance. <br /> 4. In addition, the following information must be submitted within 30 days of the removal <br /> date: <br /> (a) Analytical results, mailed from the lab <br /> (b) UST Tracking Record Sheet and Tank Hazardous Waste Manifest <br /> (c) Tank Closure Certification Form <br /> (d) Hazardous Waste Manifest for piping, rinsate, residual fuel, or waste oil receipt if <br /> applicable <br /> (e) Submit the backfill excavation certificate as required by San Joaquin County <br /> 5.
