Name of Contractor QUOTATION <br /> Contractor ' s Classification Class A <br /> License Number QUOTATION <br /> Certificate of worker ' s Compensation insurance . . . <br /> 1 ��C QUOTATION <br /> budd 4 . Tank #4 is a 5 ,000 gallon tank containing #2 Diesel <br /> fuel . This tank has never stored any other product. <br /> 5 . Lincoln Unified School District will be using <br /> California water Labs or other approved laboratory as <br /> specified in quotation to collect the soil samples . A <br /> signed authorization to release rulsults will be <br /> attached upon approval of QUOTATION. <br /> 6. Method of disposal. . . approximately 1 ,035 gallons of #2 <br /> Diesel fuel will be removed by Lincoln Unified School <br /> District. Residual fuel will be removed by contractor <br /> in accordance with QUOTATION. <br /> 7 . Tanks to be purged of flammable vapors . . . . .as specified <br /> in QUOTATION. <br /> 8 .
