Facility address and telephone number of owner (or operator <br /> if different than owner) . <br /> The tanks are located at the J. R. Simplot Company fertilizer <br /> manufacturing and agricultural chemical formulating facility at <br /> 16777 Howland Road, Lathrop, California. The telephone number of <br /> the facility is ( 209 ) 858-2511 . <br /> 2. Provide a parcel plot plan showing tank( s) to be removed and <br /> tank(s) distance from all premise septic tanks, buildings, and <br /> property lines, and all wells within 500 feet. <br /> The parcel plot plan is enclosed with this letter . No known <br /> septic tanks are located within 500 feet of the tanks. <br /> 3. Provide the name of the contractor, including contractor' s <br /> classification, license number, and certificate of workers <br /> compensation insurance. <br /> Please refer to the enclosed "California-licensed contractor <br /> questionnaire" . <br /> 4. Identify the tank(s) size and product(s) currently and/or <br /> previously stored in each tank. <br /> Tank 01 size is 10,000 gallons and previously contained diesel <br /> fuel oil. Tank 03 size is 10,000 gallons and currently contains <br /> regular leaded gasoline. <br /> S. Provide the name of laboratory that will collect soil samples <br /> and a "Release of Information Authorization" form signed by the <br /> facility owner/operator. <br /> Please refer to the enclosed "Authorization to Release Analytical <br /> Results, Geotechnical Data and Site Assessment Information" . <br />
