PAGE 2 <br /> 1. 4 Supervisor's Training: <br /> Each superintendent shall be required to familiarize <br /> themselves with state laws concerning safety, motor vehicle <br /> operation, workers' compensation and labor laws. Periodic <br /> training sessions shall be arranged to assist in this <br /> requirement. In addition, each supervisor shall conduct site <br /> safety inspections at intervals established by the director <br /> of safety. All unsafe conditions or problems that cannot be <br /> resolved should be reported to the director of safety. <br /> 1 . 5 Employee Training: <br /> Each job superintendent or foreman shall make all employees <br /> aware of job rules , emergency procedures, accident reporting, <br /> workers ' rights and so forth at the start up of the project. <br /> Each new employee shall receive the same instructions at the <br /> time he starts his job. Continuing training shall include <br /> "tailgate" meetings at least every ten (10) working days <br /> presented by the job superintendent. All Training shall be <br /> recorded on the supervisor's "Safety Meeting Records" . <br /> 1 . 6 Records: <br /> All safety activities such as training, meetings, <br /> inspections, and accident/injury investigations shall be <br /> recorded on forms specified by the safety director. <br /> 2 . 0 Accident/Claims Administration: <br /> 2 . 1 <br /> Accident reporting and claims administration is a vital part <br /> of our loss control program. Prompt reporting of any accident <br /> will assist our office staff to process claims forms. It is <br /> the responsibility of the supervisors and employees to <br /> complete the field reports and investigations, and supply <br /> that data to the office. <br /> 2 . 2 . 1 Workers' Compensation Related Accidents: <br /> 2 . 2 . 1 .1 <br /> The Employer's First Report of Injury shall be completed <br /> using the data taken from the foreman's report of injury. The <br /> Employer's First Report of Injury shall be mailed to the <br /> current workers' compensation carrier of record at the time <br /> of injury. Copy of this form shall be filed with the Fed/OSHA <br /> form and maintained for five ( 5) years. DO NOT WAIT for the <br /> doctor's report to complete the report. <br />
