Jacktone Rd., E. Lodi, California. A contract has
been let to remove the tanks from the ground and turn them in for scrap
metal. However, I am told that all fees must be submitted to the San
Joaquin Health Department prior to removing the underground tanks.
The following is a break down of the fees:
$50.00 per tank x 3 tanks x 5 years (1986 -1990) = $750.00 (County Fees)
$56.00 per tank x 3 tanks x 1 year (1990) = $168.00 (State Surcharge)
Total = $918.00
Attached please find check # 34873 in the amount of $918.00 as payment in
full for past and present fees pertaining to the underground tanks at the
above referenced location. Please provide a receipt to the submittor of
this check in order that we may retain a record of this transaction for our
permanent files.
Thank you.
RE'q y neo
ENWR MAY
03 1390
PfR11Vl ES4LrN
Sincerely,
ji/ke Paisley
UST Specialist
