The following describes the violation items you noted during your review as well as <br /> corrective actions taken: <br /> CCR 2638(a) Failed to test monitoring and leak detection equipment annually. Annual <br /> monitoring system certification and leak detector testing were last performed on 1/21/2016 <br /> ad was due by January 2017. Testing has not been completed and is two months past due. <br /> These tests are required once every 12 months. Immediately schedule these tests and <br /> provide 48 hours notification to the EHD. BP Testing and Maintenance Vendor, Gettler-Ryan, <br /> Inc., conducted post-construction Monitoring System Certification and Leak Detector Testing on <br /> 12/15/2016 following a dispenser upgrade. On 03/07/2017, BP requested from you and <br /> received your approval to use the 12/15/2016 testing event to fulfill the facility's Annual <br /> Monitoring System Certification and Leak Detector Testing requirement. Copies of the <br /> Monitoring System Certification and Leak Detector Testing results, as well as our email <br /> correspondence, are enclosed herein for your review and records. <br /> HSC 25284.2 Failed to test spill containers annually.
