PUBLIC HEALTH SERVICES pQ,u!N
SAN JOAQUIN COUNTY
G
ENVIRONMENTAL HEALTH DIVISION '° X
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 95202
2091468-3420
MEMORANDUM
Date: April 27, 2001
To: Parthian Inc.
140 Lathrop Road
Lathrop, CA 95330
From: Alison Youngblood, REHS
San Joaquin County PHS-EHD
Re: Permit Request (SR0025398) for Lathrop Chevron
San Joaquin County Public Health Services Environmental Health Division (EHD) has
performed a preliminary review of your submittal for permit to replace the monitoring
system at the above-referenced location. The following issues must be addressed prior to
further review of the permit application:
1. The sensors that are installed at the site (Gilbarco PA02650960200) are not
currently listed on the California State Water Resources Control Board's
(SWRCB) List of Leak Detection Equipment and Methods (LG 113-14). Within
15 days of receipt of this letter please submit documentation which states that
these sensors have approval in the State of California (according to LG 113) or the
sensors must be immediately replaced with an alternate model which is state
approved. According to the original installation plans the sensors that were
installed were Gilbarco PA2592000000. This discrepancy indicates that the
sensors have since been replaced without a permit from the EHD.
Please contact Alison Youngblood, REHS at 209-468-9848 with any questions.
xW I
Alison Youngbloo , REHS
cc: Image Construction, Inc.
A Division of San Joaquin County Health Care Services
